ONLINE REFERRAL REQUEST

PLEASE READ CAREFULLY BEFORE COMPLETING YOUR REQUEST:

The online referral request can be used for the following:

• Members or new hires* going to work for a Local 51 signatory contractor

• Anyone signing the books at Local 51 for the first time (for entry level choose Power Groundman or Line Clearance Trimmer Trainee

*New hires are required to come into the hall to complete new member paperwork.

Please read the instructions on the forms carefully. Important - there are two (2) forms that must be completed. After you click Submit on the first form, you will be redirected to the referral form. Both forms must be completed and you must provide accurate information or your request will be rejected.

All requests are subject to approval by the Business Representative.

Members - please note that you will need a picture of your DUES TICKET to complete your referral. If you submit anything other than your dues ticket, your request will be rejected.

You must have been in the Local 51 IN PERSON in the last year to use the online referral request. If you have not been in the Local 51 Hall in the last year, please come in person to 3171 Greenhead Drive Springfield, IL 62711 Monday through Friday, 7am to 4pm.
